How can I make a public records request?


ANSWER

Simply click on the form below if you wish to make your request in writing; complete it, and either bring to Village Hall, send via email or fax to the appropriate department. See contact us for a list of departments and contact information. You may also contact the appropriate department via telephone to make your request. Thank you.
